1. Garment Information: style, color, quantity, sizes

2. Type of Decoration: embroidery or screen printing

3. Decoration Details: locations, size, colors, number of colors (screen printing)

4. Special Instructions

5. If you have artwork attach it to your email.

Note that shopping cart  prices are for single item, embroidered articles only.
